• 1 Emergency Exam & X-ray
• 10% Off Additional Treatments
• 50% Off Custom Bleach Trays

The Perio Plan requires an initial SRP "Deep Cleaning" at an additional cost of $1,000 ($500/side).

This is not dental insurance and cannot be use in conjunction with insurance or other discounts. Plan payment is due at the time of service and will be applied to future services
